Crimea State Medical University, a leading university in Russia, was established in the year 1931. - Your completed application form including your full name, date of birth, citizenship, permanent address, passport number, phone, fax, and e-mail.
- Original of Transcript of your high school or college/university education with a complete listing of courses together with Originals of all your diplomas, certifications and licenses. But all need to be translated into Ukrainian language and legalized by the Russian embassy in your country).
- Original of your Birth Certificate that must be translated into Ukrainian language and legalized by the Russian embassy in your country.
- Copy of inner page(s) of your Passport
- The results of the physical examination must be translated into the Ukrainian language as well as legalized by the Russian embassy in your country. Also, results of laboratory test for the presence of the AIDS virus that also should be translated into Ukrainian language and legalized by the Russia embassy in your country.
- 12 photographs measuring 3×4 cm each. Also, one Photostat copy of your passport and Ukrainian visa is issued by the Russian embassy. Keep in mind that your flight ticket should be to and from to your home country.
